ABOUT LIVINGWELL SUPPORTIVE COMMUNITIES

LivingWell Supportive Communities Inc (doing business as “WALK150” and “LIVINGWELL FITNESS”) was organized as an Illinois not-for-profit corporation in October 2024 to champion the health and well-being of individuals and families by developing neighborhood-based walking clubs and advanced medical fitness centers that further the tenets of lifestyle medicine. And by collaborating with health systems and nutritious food service providers to further systematic change toward preventive healthcare, in marginalized and medically underserved neighborhoods throughout the United States. 

THE PROBLEM

Alarming Rates of Preventable Chronic Disease. The U.S. healthcare system has historically placed a disproportionate emphasis on the treatment of existing illnesses and symptom management in comparison to preventive care, resulting in increased emergency department visits, hospitalizations, and healthcare costs. According to the Centers for Disease Control and Prevention (“the CDC”), “six out of every ten adults in the United States have at least one chronic disease, and about four in ten have two or more chronic diseases”—chronic diseases are conditions that occur for one or more years and necessitate ongoing medical care, including diseases such as cardiovascular conditions, cancers, diabetes, and Alzheimer’s disease. These conditions are also among the leading causes of death, accounting for 70% of all deaths in the U.S.”   At the same time five of the top 10 leading causes of death in the US are, or are strongly associated with, PREVENTABLE and TREATABLE chronic diseases.

Ballooning Healthcare Costs. Aside from the pain and suffering associated with chronic disease, a study published in the American Journal of Health Promotion  found that “a lack of movement accounted for $192 billion in annual healthcare costs, an $80 billion increase from a similar study completed in 2015”.3 Despite spending the most on health care among developed countries, the U.S. generally has lower life expectancy, higher infant and maternal mortality, more chronic disease, and higher rates of preventable deaths than most peer nations. With the incidence of chronic disease on the rise for decades now, along with unprecedented reductions in healthcare spending, the U.S. healthcare system is in a heightened state of crisis.

Disparate Rates of Chronic Disease and Life Expectancy. African Americans experience significant health disparities compared with other populations across a range of conditions and outcomes. Research indicates that African Americans are substantially more likely to have chronic diseases such as diabetes (72% higher likelihood), obesity (23% higher likelihood), and asthma (20% higher likelihood). African Americans also face higher mortality rates from several major illnesses, including cervical cancer (52% higher), breast cancer (40% higher), heart disease (25% higher), stroke (44% higher), and prostate cancer (twice as high). In addition, African American women are three times more likely to die from pregnancy-related complications, while African American infants are 2.5 times more likely to die before their first birthday. In Chicago, these inequities contribute to major differences in life expectancy. A 2022 study by the Illinois Department of Public Health found that African American residents live an average of 11.4 years fewer than other Chicagoans. The gap is driven primarily by higher rates of chronic disease, homicide, opioid overdoses, and infectious diseases. The study also highlighted stark geographic disparities: many communities on Chicago's West and South Sides have life expectancies of 69 years or less, while some North Side communities have life expectancies ranging from 75 to 78.3 years.

Inadequate Access to Preventive Care Services. Financial, logistical, informational, workforce, and coverage barriers prevent access to effective preventive healthcare solutions, including affordable medically-integrated fitness and nutritional services.

Healthcare Workforce Shortages. As of June 30, 2024, there were 7,501 designated primary care Health Professional Shortage Areas (HPSAs) in the U.S., with nearly 75 million residents (about 22% of the population) living in those areas.” More-timely treatment can improve health outcomes.

OUR SOLUTION

Increase Access to Affordable Preventive Healthcare Services, including access to public parks, aerobic physical activities, health screenings, and medical fitness center services, where a 10-yr study published by the American Journal of Preventive Medicine in 2021 found that medical fitness center members had a 60% lower risk of all-cause mortality”. 

Further the Tenets of Lifestyle Medicine, through collaborations with local health systems, nutritious food service providers and parks to prevent lifestyle related diseases that are enhanced by inadequate nutrition, exercise and sleep. And further access to nutritious and medically tailored meals that meet the dietary needs of individuals with chronic illness.

Create AI Career Pathway Opportunities with Health Systems and Nutrition Food Service Providers, by collaborating with local health systems and food providers on career pathways in integrative healthcare AI (artificial intelligence) and culinary medicine programming, including learning, mentoring, internship, and clinical rotation opportunities for students.

PROGRAMS AND INITIATIVES

WALK150

WALK150 is a neighborhood-based walking club program that facilitates noncompetitive fitness activities, preventive healthcare education, and social engagement.  The Program is designed to improve health outcomes by encouraging and facilitating 150 minutes of weekly group walking events, virtual physical activity, health education, and social engagement for individuals of all ages and fitness levels, especially at-risk populations and individuals living with chronic illnesses (e.g., diabetes, cardiovascular disease, arthritis). WALK150 clubs are co-governed by LivingWell Supportive Communities and local Community Wellness Councils (CWC), responsible for local program oversite and community outreach. The CWC is comprised of community leaders, medical professionals and area residents.

LIVINGWELL FITNESS

COMMING SOON. LivingWell Fitness is a planned medical fitness center designed to address whole health through exercise and behavior change. Members will experience (i) a lifestyle medicine, digital fitness platform and equipment orientation; (ii) health fitness assessments that measure blood pressure, body composition, strength, balance, mobility, and cognitive health status; (iii) an AI-driven fitness prescription and an initial exercise planning consultation with a certified exercise professional (iv) progressive AI-driven exercise coaching and quarterly re-assessments with an exercise professional, and (v) medically-integrated fitness programming led by certified exercise professionals with physician oversite, including exercise pathway programs for cancer, cardiology, diabetes, pre/post-natal, pre/post-surgery, orthopedic, pulmonary, cognitive health, and weight management patients. LivingWell, leverages the expertise of its equipment partner and turnkey consultant, TECHNOGYM— a global fitness and health technology company that designs, manufactures, and provides medically integrated smart equipment w/ embedded AI and digital platforms, allowing users to track workouts, receive coaching, and sync data across devices.

Wellness Fest

From July 10 to October 9, 2026, Black Oaks Center for Sustainable Renewable Living NFP (BOC) and LIVINGWELL and will host a series of lifestyle medicine events aimed at addressing a public health crisis where too many individuals have poor dietary patterns and are not physically active enough to maintain good health. The WELLNESS FEST integrates physician-curated food markets, in-person and virtual food preparation demos, food tastings, health screenings, group walking club registration, virtual aerobic exercise coaching, health education and social connection. And features BOC's Healthy Food Hub and Rx Therapeutic Food Bag programs, created by Dr. Jifunza Wright-Carter, a family physician with 40+ years using food as medicine. The goal of WELLNESS FEST is to prevent lifestyle related diseases through healthy dietary patterns, aerobic physical activity and social connection.

Co-founder and Co-Executive for Community Development

Social entrepreneur, community organizer, real estate developer and economic development consultant, providing project visioning, financial feasibility analysis, site acquisition analysis, public approvals, architectural design management, construction management and brokerage services for medical facility, affordable housing and economic development projects. Holds a MS, Real estate Development, Columbia University; MPA, Baruch College; BS Architecture, University of Illinois-Champaign; and Certificate in Not-for-profit Management - Institute for Metropolitan Affairs, Roosevelt University. Oversees operations, fundraising and neighborhood programs. Serves as liaison to the board of directors.

Senior Consultant at Colloton + Bauer Luce and an experienced healthcare strategist with nearly two decades of leadership as Chair and Administrator of the Illinois Health Facilities And Services Review Board, a public board that facilitates the Health Facilities Planning Act and Certificate of Need approval process before building, expanding, closing, or significantly changing a healthcare facility or service. Holds a BA, Health Services Administration, Sangamon State University; and MPH, University of Illinois-Springfield. Oversees communications, brand management, government relations, health system/ Provider partnerships, and clinical program referral / membership pathways.

The former President of PFM Enterprise LLC, comprised of 5 Popeyes Chicken franchise stores; Director of Planning & Analysis w/Allied Domecq, a British based conglomerate that owned several quick service restaurant brands. Yolanda is a Registered Dietitian, faculty member and Research Manager in the Department of Family and Preventive Medicine at Rush University Medical Center, with expertise in nutrition science and its role in preventing and managing chronic disease. Extensive experience managing community-based research studies, clinical trials, and multi-disciplinary teams. Work focuses on understanding and addressing health disparities, designing and implementing research that supports diverse populations, and translating findings into actionable insights for patients, communities, and healthcare systems. BLUEPRINT FOODS (BPF) is a Chicago-based food production, co-manufacturing and commercial kitchen development company providing recipe development, menu optimization, product customization, co-packing and private label services to supermarkets, specialty retailers, restaurant chains, hospitality companies, and entrepreneurial chefs.
